Blizzard has reiterated that due to its dark nature, Diablo 4 doesn't feature a secret cow level. Fans don't care because they're looking anyway.

It didn't take long, but Diablo 4 is already being toppled by the most dedicated fans of the franchise. Although the story mode may be over for some, there's still plenty to poke around in when it comes to Sanctuary.
The game's world is gigantic, and with plenty of content yet to come, there's no sign of the fun coming to a close, regardless of your chosen class. Of course, there are secrets hiding under those tree stumps and ore veins - and one secret has been rumoured since launch. Fans think the creators of Diablo 4 are lying.
Diablo 4 devs insist there is no cow level
The concept of a "cow level" has permeated through Diablo for years, with rumours of its non-existent appearance in the first game leading to a level being developed for Diablo II, and its sequel too.
The cow level is something of meme legend in the Diablo world, and now, fans are poking around in the game looking for it in Diablo 4. Even though they expect the tradition of the Cow Level will be upheld by the team behind the game, devs are pretty staunch about the fact it isn't real.
"We've been really focused on trying to keep it as grounded as possible," says Rod Fergusson in an interview with Kinda Funny. "And because of that, there's no secret level in Diablo IV that people might be looking for."

Even in spite of the fact that the cow level has been disproven by developers, fans are still looking anyway. Diving deeper into the mythos, one wrote, "alright I've been playing Diablo IV for a while, where's the cow level at." Reference to "Oxen Gods" in Ked Bardu definitely have us suspicious.
Another shared a picture of their character approaching a cow and joked, "There is no cow level!? I WILL FIND IT ANYWAY! TAKE ME TO YOURE LEADER!!!" With all of the content that is actually free to pick up in Diablo 4, players much more compelled by the content that they actually don't have at all. We'll allow it - after all, it'll get them out of developer DMs if they're all looking for special cows.
