Many end-game activities in Diablo 4 require players to use certain items and partake in rituals to access them, and Distilled Fear is one such item in Diablo 4 that players need to craft a Nightmare Sigil. This is required to access a dungeon where players can fight the Beast In Ice boss.
The Beast In Ice is an end-game boss that drops items like Frostburn, Mother's Embrace, Fists of Fate, Tassets of the Dawning Sky, Skull Torch, Fell Steed and many other lucrative drops that draw players' attention.

How to get Distilled Fear
In Diablo 4, you can get the Distilled Fear by farming Tier 30 or more Nightmare Dungeons at World Tier 4, which you can reach by clearing the Fallen Temple Capstone Dungeon on World Tier 3.
Similar to many other summoning items, Distilled Fear has no guaranteed drop, as the drop depends on RNG. Another thing to note is that while you can get Distilled Fear while farming Tier 30 or more Nightmare Dungeons, going for a higher tier dungeon will not increase the drop rate.
So, if you seek challenge, you can go for higher tier Nightmare Dungeons. For regular players looking to get the Distilled Fear as soon as possible, we recommend farming the Tier 30 Nightmare Dungeons.
How to use Distilled Fear
Distilled Fear is a summoning item in Diablo 4 that you can use to craft the Glacial Fissure Nightmare Sigil to access the Glacial Fissure Nightmare Dungeon located in the southwest part of Fractured Peaks region in the Desolate Highlands.
How to craft Glacial Fissure Nightmare Sigil
You can craft the Glacial Fissure Nightmare Sigil using nine Distilled Fear and 250 Sigil Powder. You can get the latter by completing Nightmare Dungeons and salvaging unwanted Nightmare Sigils at the Occultist. Once you have the required items, head to any Occultist to craft the Glacial Fissure Nightmare Sigil.
