Firstly, you can avoid the Diablo 4 Holy Chalice bug altogether by completing all Sister Octavia quests in the same World Tier

If you are already encountering the bug, you will need to complete a quest given by the NPC next to the Kyovashad Waypoint. This reportedly fixes the issue once the quest is complete, meaning the Holy Chalice won't continue to spawn in. 

If none of these fixes are working, then you will need to wait for an official patch, as it's likely that Blizzard is already aware of the issue and working on solving it.

Diablo 4 Holy Chalice bug causes

The Diablo 4 Holy Chalice bug seems to occur for players who complete quests in the Sister Octavia questline in different World Tiers. The related quests are:

  • Malady of the Soul
  • Depths of Despair
  • Faith In Blood

The bug results in the Holy Chalice item (which is key to the questline) appearing at the gates of Kyovashad, or in your inventory even when it should no longer be available. 

Even if you pick the item up, once you reset the game, it will appear right back in the same spot again. Luckily, it seems like a fix has been discovered.
